Online Scheduling Options
This feature allows you to request a consultation or treatment time, but does not process cancellations or reschedule. Please call our office at 563-582-7878 if you need to cancel or reschedule an existing appointment. Online requests will send notification to our office at which time they will be reviewed by our staff and approved. You will receive confirmation during our normal business hours and any after hours requests will be confirmed on the next business day.
Previous Patients
If you have been an Elements Acupuncture patient and it has been more than 12 months since your last treatment, please call our office at 563-582-7878 to schedule an appointment.
Current Patients
For those patients that are currently on a comprehensive treatment plan, wellness program or have had treatments in the preceding 12 months, please complete the form below to request your next appointment.
New Patients
